Service Locations

Occupational Therapy services are provided in the home in the following areas: downtown, the northside of Chicago, and near north suburbs.  Physical Therapy services are provided in the home in the following areas: downtown and the western suburbs including parts of Naperville, Lombard, Wheaton, Winfield and Glen Ellyn and surrounding areas.

Telehealth options are also available in certain situations, but often will follow an in-home assessment.  Contact us to learn more 

Insurance

Vitalize Home Therapy PLLC is a participating provider with Medicare Part B and  Blue Cross/Blue Shield of Illinois PPO plans and is an out of network provider with many Medicare Advantage plans for medically necessary Occupational Therapy and Physical Therapy services.  

Private Pay Options

Vitalize Home Therapy PLLC offers private pay wellness and consultative services.

Getting Started

You will need a referral or order from your physician for “Outpatient Occupational Therapy” or Outpatient Physical Therapy.  We will then verify your insurance benefits before scheduling your initial in-home assessment.
